Pursuant to Art. 13 GDPR* we would like to inform you that:

1) the personal data administrator is the Jan Kochanowski University in Kielce, ul. Żeromskiego 5, 25-369 Kielce, e-mail: kancelaria@ujk.edu.pl (hereinafter referred to as UJK),

2) a Data Protection Inspector has been appointed at UJK to supervise the correctness of data processing, who can be contacted at the following e-mail address: iod@ujk.edu.pl,

3) personal data will be processed in order to support the E+DIETing LAB project for statutory, statistical and archival purposes,

4) personal data will be processed pursuant to Art. 6 section 1 letter c GDPR (necessity to fulfill the legal obligation imposed on UJK) in connection with the Act of July 20, 2018 - Law on Higher Education and Science (Journal of Laws of 2020, item 85, as amended) and the related acts issued therein implementing regulations, in particular regarding studies,

5) providing personal data is voluntary, but failure to provide them will result in the inability to participate in the Project and its further stages

6) data recipients may be entities authorized under the law and entities with which UJK concludes appropriate contracts for the provision of external services,

7) personal data will be stored for a period of 6 months from the date of completion of the Project

8) the data subject has the right to access his or her personal data and receive a copy thereof, rectify it, delete it (if the data is not processed in order to fulfill the obligation arising from legal provisions) or limit their processing,

9) the data subject has the right to lodge a complaint with the President of the Personal Data Protection Office.
